Apple Watch will happily show you a VO2 max number, label it "Cardio Fitness," and tell you whether it is low for your age. What it does not explain is where that number comes from, why it sometimes refuses to appear at all, or how much weight it deserves.

Here is the honest version: your watch is not measuring your VO2 max. It is estimating it from how your heart responds to a known workload, using a model. That estimate is useful — but only if you know what feeds it and what breaks it.

What "Cardio Fitness" on Apple Watch actually is

Apple calls it Cardio Fitness. It is a VO2 max estimate: the maximum amount of oxygen your body can use during exercise, expressed in millilitres of oxygen per kilogram of body weight per minute (mL/kg/min). It is one of the better single indicators of aerobic fitness, and it tends to track with long-term health outcomes.

A real VO2 max measurement means a lab test — a mask over your face, a treadmill or bike ramping up until you cannot continue, and direct analysis of the gas you breathe out. That is the reference standard your watch is trying to approximate from your wrist.

How Apple Watch calculates VO2 max

Apple Watch Series 3 or later records an estimate using its heart and motion sensors during specific workouts. The logic is straightforward: motion and GPS tell the watch how hard you are working, the optical sensor tells it how hard your heart is working to do it, and the relationship between those two is what a VO2 max model runs on. A lower heart rate at a given pace means a higher estimate.

Alongside the sensor data, Apple factors in your:

  • age
  • sex
  • weight
  • height
  • medications that might affect your heart rate

Two consequences follow immediately. First, a stale weight in your Health profile skews the result, because VO2 max is expressed per kilogram of body weight — the number is literally divided by it. Second, if you never told Health about heart-rate-affecting medication, the model does not know to adjust.

Apple supports a VO2 max range of 14–65 mL/kg/min, and cardio fitness classifications are available to users 20 years or older. If your true value sits outside that band — which it might for a well-trained endurance athlete — the watch cannot show it.

Only three workout types count

This is the single most common source of confusion. Apple Watch estimates cardio fitness during:

  • Outdoor Walk
  • Outdoor Run
  • Hiking

In Apple's words, indoor workouts, including ones that require gym equipment, do not count towards your cardio fitness estimate. Push workouts are not supported either.

So the treadmill session does not contribute. Neither does the spin bike, the rower, the elliptical, or the strength session. If you train hard but train indoors, your watch can sit for weeks without updating a number — and that is the system working as designed, not a fault.

The reason is mechanical rather than arbitrary. The model needs a trustworthy external workload to pair with your heart rate. Outdoors, GPS and motion give it real speed and elevation. On a treadmill, the watch cannot see the belt speed or the incline, so it has no reliable workload to anchor the estimate to.

Why you do not have a VO2 max reading yet

Apple says it can take at least 24 hours of wearing your Apple Watch, followed by several Outdoor Walk, Outdoor Run, or Hiking workouts plus passive measurements, before you get an initial estimate. Work through this list before assuming something is broken:

  • You are on Series 3 or later.
  • You have recorded qualifying outdoor workouts — not indoor ones.
  • The walks and runs were long and steady enough to give the model a usable stretch of data.
  • Your health profile has age, sex, height, and weight filled in.
  • You are 20 or older, for the classification to appear.
  • The watch is worn snugly enough for the optical sensor to hold a clean signal.

How accurate is Apple Watch VO2 max?

The honest answer has two parts, and most articles only give you one.

As an absolute number, treat it as approximate. It is a submaximal estimate produced by a model, not a measurement of the gas you breathe. Compared against a lab test it can be out by a meaningful margin in either direction, and the gap is usually larger at the extremes — very fit people often see their watch under-read, partly because the supported range tops out at 65.

As a trend, it is genuinely useful. Whatever bias the model has for you personally tends to be consistent, so the direction of travel over months is far more informative than the value on any given day. If the number climbs across a training block, something real improved.

Specific things that push the estimate around:

  • Hills and wind. The model reads pace as workload. A headwind or a climb makes your heart rate high for the pace, which reads as lower fitness.
  • Heat, dehydration, poor sleep, illness, alcohol, caffeine. All of these raise heart rate at a given effort, and all of them push the estimate down.
  • Stop-start routes. Traffic lights and junctions break the steady pace-to-heart-rate relationship the model wants.
  • A loose watch. Optical heart rate is most fragile during movement; a bad signal feeds bad data into the estimate.
  • Heart-rate-limiting medication. Apple states directly that certain conditions or medications that limit your heart rate might cause an overestimation of your VO2 max — a lower heart rate looks like better fitness to the model. Beta blockers are the common example.

None of this makes the metric useless. It makes it a trend line with noise, which is how it should be read.

Why your VO2 max suddenly jumped or dropped

If your number moved several points overnight without any change in training, the cause is usually not your fitness. Apple includes an update mechanism: if the Health app detects that your previous cardio fitness estimates could be more accurate, you get the option to update your data. In Cardio Fitness, tap Update under Update Your Data, and a Cardio Fitness Data Updated notification then shows how your estimates changed.

That process can rewrite historical estimates, so a step change in the chart is often a recalculation rather than a physiological event. Correcting a wrong weight or adding a medication in your health profile can do the same thing.

How to get a more reliable estimate

  • Give it a clean run. A 20-plus-minute Outdoor Run or brisk Outdoor Walk at a steady effort, on flat ground, on a route without constant stopping.
  • Wear the watch properly. Snug, above the wrist bone, not sliding around. Optical sensors need contact.
  • Keep your health profile current. Update your weight when it changes — the number is per kilogram, so an old weight biases every estimate.
  • Record medications that affect heart rate, so the model can account for them.
  • Compare like with like. Judge the trend across similar routes and similar conditions, not a hilly summer run against a flat winter one.
  • Calibrate your watch for improved workout and activity accuracy if your pace data looks wrong.

Where to find your number

Open the Health app on your iPhone or iPad, tap Search (or open the sidebar on iPad), tap Heart, then Cardio Fitness. You can switch between Day, Week, Month, 6 Months, and Year views, and tap Show All Cardio Fitness Levels to see where you sit relative to others of your age and sex.

The comparison chart is worth a look, but hold it loosely — you are comparing a modelled estimate against population averages, so a category boundary is not a hard line.

What to actually do with the number

VO2 max responds to training, and the training that moves it is not mysterious: a large base of easy aerobic work plus a small amount of genuinely hard work. If your estimate has been flat, that mix is the first place to look — most people doing "cardio" spend their time in the middle ground that is too hard to recover from and too easy to drive adaptation.
